Testing Proposal Request
This starts as a proposal request so CHA can confirm athlete count, venue, timing, testing format, report needs, pricing, and the best follow-up training path.

MyCHA Account

CHA schedules camps and training based on confirmed ice allocations, facility availability, staff availability, registration numbers, age, level of play, safety, and group fit. Published times, groups, coaches, dressing-room assignments, off-ice blocks, and daily programming may be adjusted when needed.
Families are responsible for checking the registration email, MyCHA notices, and any CHA schedule updates before each camp day. Friend or teammate requests may be considered, but are not guaranteed. Missed time, late arrival, early pickup, illness, injury, travel, suspension, or family schedule conflicts do not create a make-up, refund, exchange, or credit.
All CHA camp, program, training, testing, and product purchases are final once payment is made. There are no refunds, cancellations, exchanges, transfers, make-ups, or credits unless CHA confirms an exception in writing or applicable law requires it.
If CHA must cancel or materially reschedule a full program for operational, facility, safety, weather, or staffing reasons, CHA will contact affected families with the available next steps. Refund decisions, if any, are made by CHA, not through a self-serve parent cancellation process.
Parents and guardians must disclose allergies, medications, injuries, medical conditions, behavioural or accessibility needs, emergency contacts, and any participation limits that may affect the athlete's safety or care. CHA relies on the information submitted in the registration and MyCHA account.
Medical information is used only for program safety, emergency response, and required administration. CHA may share necessary information with CHA staff, facility staff, emergency contacts, and medical responders when reasonably needed for the athlete's care. Parents or guardians must provide written instructions in advance for medication, emergency medication, or any special care plan. CHA may decline or pause participation if required safety information is missing or if participation would create an unreasonable safety risk.
CHA will release a child only to the parent or guardian on file, or to another adult authorized by that parent or guardian in writing. CHA may ask for photo identification before release. Pickup changes must be sent by the parent or guardian on file before pickup.
A child may leave independently only if the parent or guardian gives written permission in advance, including the athlete's name, date, and the approved release instructions. Verbal instructions from a child, teammate, or unauthorized person are not enough. Parents and guardians are responsible for timely pickup at the end of the scheduled program day.
By registering, the parent or guardian confirms that the athlete may participate in hockey, skating, off-ice training, testing, and related activities, and accepts the normal risks of athletic participation. Families must follow CHA, facility, dressing-room, safety, and conduct rules.
CHA may use photos or video for coaching, program administration, and CHA promotional content unless the parent or guardian sends a written opt-out before the program begins. CHA may remove an athlete from activity for unsafe conduct, facility rule issues, bullying, harassment, or behaviour that creates a safety concern. Removal for conduct does not create a refund, credit, or make-up right.
CHA athlete testing reports are provided for athlete development, family review, team service delivery, and authorized coaching use. CHA owns and controls its testing system, benchmark tables, calibration logic, standards, report templates, aggregated or anonymized performance standards, and internal methodology.
Users may not copy, scrape, reproduce, extract, resell, redistribute, publish, upload, train models on, reverse engineer, or build a competing system from CHA testing reports, athlete testing data, standards, benchmark data, report templates, or app content without written authorization from Central Hockey Academy Ltd.
